General Liability Release: I and my parent/guardian fully understand that I will be engaging in activities that involve risk of serious injury, including permanent disability or death, which may result from my conduct, conduct of others, the rules of play, and the condition of the premises and of any equipment used, and from other unknown risks not reasonably foreseeable at this time. With a full understanding of risk, I and my parent/guardian assume all risk and hereby release, discharge, promise to waive, release, absolve indemnify and not sue Brewer Travel Basketball Club, it’s board of directors, officers, coaches, and including owners of the premises used to conduct all basketball activities from all liability to me, my parents/guardians, my next of kin and my heirs. I agree to pay for any and all damages to any property caused by me negligently, willfully, or otherwise. I also agree that Brewer Travel Basketball Club and their agents, sponsors, may use my child’s photograph in future promotions in print, broadcast media or posted on the internet as deemed appropriate for the promotion of the Brewer Travel Basketball Club.

Medical Release/Liability Waiver: I consent to medical treatment for emergencies that occur during or are related to my participation in the Brewer Travel Basketball Club where I am unable to consent for such treatment. I understand that I am solely responsible for any medical, health or personal injury costs relating to my participation in the Brewer Travel Basketball Club Program. I understand that I am strongly encouraged to have a medical physical examination and purchase health insurance prior to any and all participation in the Brewer Travel Basketball Club program.
